Une évaluation avant toute proposition de traitement

Une douleur au dos ou une douleur qui descend dans la jambe peut avoir plusieurs origines. Une décompression ne doit donc pas être proposée uniquement à partir d’un symptôme, d’une radiographie ou d’une IRM prise isolément.

Comprendre

Revoir le début des symptômes, leur trajet, les positions aggravantes, le sommeil, le travail et les traitements déjà essayés.

Examiner

Observer la mobilité, la marche, la force, la sensibilité et les réactions aux mouvements dans les limites du champ de pratique.

Vérifier l’admissibilité

Comparer les symptômes aux examens disponibles et rechercher les contre-indications ou les signes nécessitant un médecin.

Réévaluer

Suivre la douleur, la fonction et la tolérance, puis modifier ou interrompre l’approche si l’évolution n’est pas favorable.

Important. Une faiblesse progressive, une perte de contrôle urinaire ou intestinal, une anesthésie de la selle, de la fièvre, un traumatisme important ou une aggravation rapide nécessitent une évaluation médicale urgente.

Qu’est-ce que la décompression neurovertébrale motorisée?

La décompression neurovertébrale est une forme de traction mécanique contrôlée appliquée à la colonne lombaire ou cervicale au moyen d’une table spécialisée. Le protocole peut varier selon l’appareil, la région traitée, la force utilisée, la durée et la réaction du patient.

L’objectif clinique déclaré est de modifier temporairement les contraintes mécaniques ressenties au niveau de la colonne et d’améliorer la tolérance fonctionnelle chez certaines personnes. Elle ne « remet » pas automatiquement un disque en place, ne garantit pas sa régénération et ne convient pas à toutes les causes de douleur.

Niveau de preuve et prudence. Les recommandations NICE ne conseillent pas la traction pour la lombalgie avec ou sans sciatique. Le terme « décompression » pouvant recouvrir des appareils et protocoles différents, le patient devrait recevoir une information claire sur les données disponibles, les limites, les coûts et les autres options.

Dans quelles situations cette approche peut-elle être discutée?

La présence d’une hernie discale ou d’un changement dégénératif sur une image ne suffit pas à déterminer le traitement. Les symptômes, la fonction, l’examen et l’évolution doivent être concordants.

Situations à signaler

Fracture récente, chirurgie vertébrale récente, ostéoporose importante, maladie inflammatoire active, grossesse ou dispositif médical implanté.

Orientation médicale

Suspicion d’infection, de tumeur, de fracture, déficit neurologique progressif ou symptômes généraux inexpliqués.

Arrêt et réévaluation

Douleur qui descend davantage, engourdissement croissant, faiblesse nouvelle, malaise ou aggravation durable après une séance.

Aucune garantie de résultat. Une amélioration ne peut pas être promise. L’absence de progrès mesurable doit conduire à une réévaluation du plan et, au besoin, à une orientation vers un autre professionnel.
